Autonomy begins after the problem has been chosen, and choosing it is a human act. A vast space of possible targets, diseases, and mechanisms exists, but only some are worth pursuing. That selection rests on patient need, prior evidence, feasibility, and values about who should benefit — none of which can be read off a dataset. AI can rank and estimate within a defined space, but it cannot decide which space deserves the effort.
