The loop at tissue scale
Sustained NF-kB and inflammasome activity maintain IL-1 family cytokines and TNF. These recruit leukocytes whose ROS and proteases create new damage-associated molecular patterns, which re-prime the same signaling. The loop is self-sustaining and no longer requires the original insult.
Fibrosis as an altered set-point
Continuous MMP release degrades normal matrix as well as damaged matrix, while repair deposits collagen faster than it is cleared. The tissue stabilizes at a stiff, collagen-rich state. Mechanical tension on fibroblasts promotes further collagen deposition, so the new set-point reinforces itself.
Same molecules, different verdict
NF-kB and MMP activity are protective acutely and harmful chronically. Classification must therefore name the duration and ask whether the loop can be switched off, not just which pathway is active.
